Application Form Submission
Click the button above to download the application form. Either print the form and fill by hand or use a PDF editor to fill out the form digitally. Submit the PDF form by completing the form below.
Instructions
Apple/Mac
Or, follow the instructions below.
- Right-click the "Download Application" button.
- Chrome Browser: Select "Save Link As".
Safari Browser: Select "Download Linked File As". - Save the file to the “Downloads” folder.
- Open Finder
- Navigate to the “Downloads” folder
- Right-click the application and open with Preview or your preferred PDF editing software.
- In Preview, select individual fields by hovering and clicking.
- To Digitally Sign the form, click the markup button (
) in the top right to bring up additional options.
- Select the signature button (
) from the markup toolbar.
- Using your laptop's trackpad, draw your signature with your finger, and select done. Alternately, select “camera” at the top then sign a blank piece of paper and hold it up to the computer’s camera.
- Click the signature you just created to add it to the document.
- Drag to move into position and use the blue handles to resize.
- Once complete, save the document and return to the website.
- Enter your full name, email and phone number in the upload form.
- Click “Choose File” and select the signed PDF.
- Click the checkbox next to “I’m not a robot”. If it does not turn into a green check, follow instructions in the popup.
- Click Submit.
iOS/Apple Mobile Devices
For these instructions, we are using an app called PDF Expert available in the Apple app store. This a free app with paid extensions. The paid extensions are not required to complete these steps.
- Tap “Download Application”
- Click the Share icon at the bottom.
- Select "Copy to PDF Expert"
- Fill the Form using PDF Expert. To Sign, tap the pen icon at the top and sign using your finger.
- When complete, tap the back arrow (<) in the top left to save. Then return to the website.
- Select “Choose File”
- Choose “Browse”
- Choose “Browse” in the bottom right.
- If “iCloud Drive” opens, choose “Locations” in the top left to browse other locations.
- If PDF Expert is available, select PDF Expert. If not, continue to the next step.
- If it is not available, select the three dots in the top right. Select “edit” and turn PDF Expert on.
- Select the completed form, complete the remaining fields on the Just Like Family website and tap submit.
PC/Windows
- Right-click the "Download Application" button.
- Select "Save Link As".
- Save the file to the “Downloads” folder.
- Open the file explorer.
- Navigate to the “Downloads” folder
- Double click the file to open in Microsoft Edge.
- Fill out the form by clicking on each field or applicable check boxes.
- To sign, select the “Draw” button at the top and draw in your signature with your mouse or trackpad. Click “Draw” again to exit draw mode and continue filling out the form.
- When finished, select the save icon in the top right. Navigate to “Downloads” and save the form, replacing the original.
- Return to the website to upload the form.
- Enter your full name, email and phone number in the upload form.
- Click “Choose File” and select the signed PDF.
- Click the checkbox next to “I’m not a robot”. If it does not turn into a green check, follow instructions in the popup.
- Click Submit.
