Application Form Submission

Click the button above to download the application form. Either print the form and fill by hand or use a PDF editor to fill out the form digitally. Submit the PDF form by completing the form below.

  1. Right-click the "Download Application" button.
  2. Chrome Browser: Select "Save Link As".
    Safari Browser: Select "Download Linked File As".
  3. Save the file to the “Downloads” folder.
  4. Open Finder
  5. Navigate to the “Downloads” folder
  6. Right-click the application and open with Preview or your preferred PDF editing software.
  7. In Preview, select individual fields by hovering and clicking.
  8. To Digitally Sign the form, click the markup button () in the top right to bring up additional options.
  9. Select the signature button () from the markup toolbar.
  10. Using your laptop's trackpad, draw your signature with your finger, and select done. Alternately, select “camera” at the top then sign a blank piece of paper and hold it up to the computer’s camera.
  11. Click the signature you just created to add it to the document.
  12. Drag to move into position and use the blue handles to resize.
  13. Once complete, save the document and return to the website.
  14. Enter your full name, email and phone number in the upload form.
  15. Click “Choose File” and select the signed PDF.
  16. Click the checkbox next to “I’m not a robot”. If it does not turn into a green check, follow instructions in the popup.
  17. Click Submit.
iOS/Apple Mobile Devices

For these instructions, we are using an app called PDF Expert available in the Apple app store. This a free app with paid extensions. The paid extensions are not required to complete these steps.

  1. Tap “Download Application”
  2. Click the Share icon at the bottom.
  3. Select "Copy to PDF Expert"
  4. Fill the Form using PDF Expert. To Sign, tap the pen icon at the top and sign using your finger.
  5. When complete, tap the back arrow (<) in the top left to save. Then return to the website.
  6. Select “Choose File”
  7. Choose “Browse”
  8. Choose “Browse” in the bottom right.
  9. If “iCloud Drive” opens, choose “Locations” in the top left to browse other locations.
  10. If PDF Expert is available, select PDF Expert. If not, continue to the next step.
  11. If it is not available, select the three dots in the top right. Select “edit” and turn PDF Expert on.
  12. Select the completed form, complete the remaining fields on the Just Like Family website and tap submit.
PC/Windows

  1. Right-click the "Download Application" button.
  2. Select "Save Link As".
  3. Save the file to the “Downloads” folder.
  4. Open the file explorer.
  5. Navigate to the “Downloads” folder
  6. Double click the file to open in Microsoft Edge.
  7. Fill out the form by clicking on each field or applicable check boxes.
  8. To sign, select the “Draw” button at the top and draw in your signature with your mouse or trackpad. Click “Draw” again to exit draw mode and continue filling out the form.
  9. When finished, select the save icon in the top right. Navigate to “Downloads” and save the form, replacing the original.
  10. Return to the website to upload the form.
  11. Enter your full name, email and phone number in the upload form.
  12. Click “Choose File” and select the signed PDF.
  13. Click the checkbox next to “I’m not a robot”. If it does not turn into a green check, follow instructions in the popup.
  14. Click Submit.
