All inquires regarding your application should be directed to the Caregiver Relations Department at JLFHC which is open during regular business hours, Monday through Friday 9:00 am to 5:00 pm. Thank you for your interest in registering with Just Like Family Home Care (JLFHC) Nurse Registry as an Independent Contractor. Please complete all sections of the application to be considered for an interview to register. We will contact you for an appointment during the next open registration time or if we have an immediate need for your skill level or schedule availability.
When an appointment time is set please provide originals of the following:
Please provide a physical dated within the last 6 months before registering with JLFHC. The physical must state that you appear to be free from communicable disease and that you are in good health, sufficient to provide services to individuals with compromised health.
The Agency for Health Care Administration (AHCA) requires that all Companions, HHA, CNA, LPN, and RN caregivers registered with JLFHC must have current Level 2 background check with a status of “Eligible”. If you have not had a Level 2 Background Check conducted within the past 5 years; arrangements must be made through an agency or registry to have an electronically submitted screening through a LiveScan vendor approved by AHCA. Applicants are responsible for the cost of any required screenings.
All Companions, HHA, CNA, LPN, RN caregivers registered with JLFHC will need to have their own policy for Malpractice Insurance. If you already have a current policy – a copy must be given to JLFHC at the time of registration. If you do not have coverage; JLFHC will provide you with the information you will need to purchase the malpractice insurance and complete your registration so you may begin receiving client referrals from JLFHC.
